Smart Forms (ACT)

The Smart Forms have a newer User Interface (UI), more functionality, etc. compared to its previous version. All forms allow administrators to deliver and collect custom information to users through MOC requirements, or ACT Action Item & Event Forms. 

How do Smart Event Forms differ from Legacy Forms

  • Smart Forms will be the only version to receive updates such as new enhancements going forward. 
  • Smart Forms have a new cleaner, minimalistic look and feel. 
  • All fields are now automatically reportable with autogenerated report label names. 
  • Smart Forms introduce new fields such as; Date Field, Number, Autocomplete, Rich Text Editor, Header, Hidden Input and Divider.
  • Conditional Logic is only available for Smart Forms.
    • Conditional logic allows administrators to configure certain sections or actions to be available or unavailable based on how users are answering the form.
    • This provides a more streamlined, condense form where users do not have to answer sections that do not apply to the MOC, Event or Action Item. 

Form Designer V2 Properties

Common properties of all field types:

  • Authors can drag and drop each field type into a section on the form to determine the layout of the form. To view or change the field properties of an existing field, select the Pen icon to edit it. Select the Trash icon to remove the field. 
  • All fields are reportable by default. A reportable field name is autogenerated to mimic the field name. (It will not autogenerate if there are special characteristics.)
  • All field types can have conditional logic attached to them, aside from the Divider field. 

Common properties of all data field types (text box, text area, radio buttons, checkboxes, and drop-down menu):

  • Any of these fields can have a default text or value. For radio buttons, checkboxes and drop-down menu fields, defaults are selected in the field options section at the bottom.
  • Any of these fields can be made a Required field. A required field is marked with a red asterisk.
  • Any field can have a Help Message to provide instructions to users about completing the field. A help icon is shown next to any field containing a help message, and the message is displayed when hovering over the icon.

Common properties of fields that have field options (radio buttons, checkboxes, and drop-down menu):

  • To reorder a field option, select it (by clicking anywhere in the grey area) and drag it to the correct order position.
  • For radio buttons and check boxes only: Field options can be arranged to be "inline" (which will stack the options horizontally rather than vertically).
  • For radio buttons and check boxes only: Field options can have an attached Comment text box. This allows user to enter optional qualifying comments when selecting a particular option.
    • Text Field: Populates an open response field typically leveraged for short-ended questions. 
    • Date Field: Allows users to add a selectable calendar for picking dates.
    • Number: Allows users to add a numeric only field. 
    • Radio Button: Allows users to only check one of the options displayed.
    • Checkbox: Allows users to check one or more of the options displayed.
    • Dropdown: Allows users to select one value from a list of drop-down field options.
    • Autocomplete: Acts as a customizable search option. You may tie certain actions or recommendations to questions similar to a form tree. 

You may change the customizable search list to search for active users

    • Text Area: Allows users to enter up to 20 lines of text up to 100 characters wide. Typically leveraged for longer open-ended questions. 
    • Header: Provides a Header for the form.
    • Label: Can be used a title or instructions for a section.
    • Hidden Input: Acts a spacing tool between sections. 
    • Divider: A solid, dashed or hidden (blank space) horizontal line can be used between fields.
    • Media: Allows users completing the form to upload documents directly to it. 

How to Create a Smart Form

  1. Navigate to Settings > Forms.
  2. Select New
  3. Select Smart Version
    1. In ACT you will need to select a Category from the category drop-down menu.
    2. Event forms are associated to categories, therefore, when creating an Event a category must be selected and the available forms for the category will be available for users to select.
  4. Enter the Title for the form.
  5. Description, Header and Footer are optional fields that can be used to further describe the form.
  6. Deselect the Active checkbox if you wish to inactivate this form. Typically done if you do not want the form to be accidentally used when building it. 
  7. Enter the correct Company and Division.
  8. Select Public if you would like other Divisions to be able to use and copy this form. 
  9. Select Save.

How to Build your Form

When selecting layout, if the form designer does not appear, please be sure your pop-up blockers are disabled.

  1. Navigate to Settings > Forms > select the Form Title
  2. Select Layout
  3. From here you can drag and drop any of the fields, defined above, to their correct orientation.
  4. After filling in the necessary information in the fields you dragged over, exit the Form Designer.
  5. Click Preview to view your form as users completing it would.

Our form builder will automatically save all changes.

How to Configure Conditional Logic on an existing Smart Form

Your form must be a Smart Form, it cannot be a Legacy Form to use conditional logic. 
  1. Navigate to Settings > Forms > select the Form Title
  2. Select Layout
  3. From here you will need to have a field in mind that your conditional logic tied to. We will call this the Parent field. 
  4. Next, navigate to a field you want to be available or unavailable based on the selections of the Parent Field. We will call this the Child field.
  5. Click the Pen icon of the Child field. 
  6. Select the Conditional Logic Toggle.
  7. From there you will have the option to show or hide the section when All or Any of the rules you configure match
  8. Select the desired functionality you wish to have from those drop downs
  9. Next, you will configure the rule(s) in which the above conditional logic adheres to. 
    1. To do this, you will need to select the Parent field from the drop down.
    2. Then, you will need to choose when the show/hide functionality will be applied.
      1. If your Parent field is a section with selectable options (radio buttons, checkboxes, etc), you will have these choices from the drop down: is selected, is not selected, is shown, is hidden. 
      2. If your Parent field is an open-ended section (text field, text area, etc.) you will have these choices from the drop down: is shown, is hidden, is equal to, is not equal to, contains and does not contain.
    3. Once you choose when the conditional logic will be applied, next you must tie that selection to a user response. The available responses displayed will be based on the options in your Parent field.
  10. You may repeat steps 1-3 to configure as many rules as needed. 
Smart Form changes are saved automatically. Select Preview to view and test the conditional logic configured.
